Position Summary

We are seeking an experienced MDS Coordinator (RN) to ensuring accurate and timely completion of MDS assessments in compliance with state, federal, and Medicare guidelines.

Key Responsibilities

• • Complete and submit MDS assessments accurately and on time

• • Ensure compliance with CMS, Medicare, and Medicaid regulations

• • Coordinate with the interdisciplinary team for care plan development

• • Review and monitor documentation to support reimbursement accuracy (PDPM)

• • Participate in care plan meetings and QA processes

• • Track and manage assessment schedules and deadlines

• • Educate staff on documentation and regulatory requirements

• • Collaborate with nursing, therapy, and administration teams

Qualifications

• • Active RN license in South Carolina (in good standing)

• • Prior MDS experience required

• • Strong knowledge of PDPM, Medicare/Medicaid, and long-term care regulations

• • Experience in skilled nursing facility (SNF) setting preferred

• • Excellent attention to detail and organizational skills

• • Strong communication and teamwork abilities
